FAQ
Is this a permit-required confined space program template or just a permit?
Both. You get the written entry procedure (Sections 1 to 14) and the entry permit, plus the logs, evaluations and records a program runs on. The procedure covers definitions, roles, identifying and classifying spaces, preparing the space, the permit, the entry itself, rescue, contractors, training, records and program review.
Can I edit the confined space permit template in Word?
Yes. It's a .docx, so there's no app and no login. Every site value sits in one table in Section 3, and a value you set once carries through the procedure. Write-in rows are at least 0.30 in tall so they work for handwriting on printed forms.
What does the federal rule say an entry permit has to identify?
29 CFR 1910.146(f) lists what the entry permit identifies, starting with the permit space, the purpose of the entry, and the date and authorized duration. The rule also says to keep each canceled permit for at least 1 year for the program review (1910.146(e)(6)). Read the rule yourself and check the permit against it with your competent person. The kit doesn't decide that for you.
Does it include a rescue service evaluation?
Yes, form CSE-7. Under 1910.146(k), an employer that designates rescue and emergency services has to evaluate them. Your site still writes its own rescue plan and picks its own rescue service.
Can I use it as a confined space permit to work?
This kit keeps the entry permit on its own form. If you also use the NightWarder permit to work kit, the PTW permit links to the entry permit by number.
Is there a free confined space entry permit template?
Not from us. The kit is $79. The filled example inside shows how a completed permit looks at a made-up site (Example Plant, Unit 2, V-99 separator, 2099 dates).
