Permit to work template and procedure, in Word

Blank template. $79 · Your file is emailed within about an hour of purchase · 7-day refund

In short

A safe work permit template on its own doesn't tell anyone who can issue it, how long it runs, or what happens at shift change. This kit pairs the permit with a written procedure and the forms that hold it together, for maintenance, contractor and other non-routine work at an industrial site. Your site decides which work needs a permit and who signs. Your competent person reviews it before it goes live.

What's in the kit

Word file, 51 to 52 pages including the filled example. $79, emailed within about an hour of purchase, 7-day refund. Licensed to one organization for one site.

FAQ

What's the difference between a permit to work form and a permit to work procedure?

The form is the PTW-1 permit people sign in the field. The procedure (Sections 1 to 16) says when a permit is needed, how the job is planned, how to stop, suspend, restart and cancel, how shift handover and close-out work, and how changes and emergency work are handled. You get both.

Can I use it as a general safe work permit template?

Yes, that's what it's for: maintenance, contractor and other non-routine work. The permit matrix (PTW-8) is where your site writes down which jobs need which permit.

Does it replace hot work, confined space or lockout permits?

No. The permit to work links those records by number, so the specialist permits stay on their own forms. Lockout, for example, still runs under your energy control procedure. The federal lockout rule, 29 CFR 1910.147(c)(4), says energy control procedures are to be developed, documented and used.

Does it work for contractors?

PTW-7 is a contractor work coordination form, and the procedure has a section on contractors and other employers.

Is it editable in Word?

Yes, it's a .docx with no app or login. Every site value sits in one table in Section 3 and carries through the procedure.

Is the filled example a real permit?

No. It shows a made-up job at Example Plant, Unit 2 (V-99 separator, 2099 dates) so you can see how the sheets fit together.

Blank template only. It isn't a legal document, a filled permit, a risk assessment or approval by any regulator. Your site adapts it, checks it against the rules that apply to you, and has its competent person review it before use.
